Some of the eye specialists in my practice use a pinhole occluder to measure monocular visual acuity. I don’t bother. Should I?

When I was training in optometry school marks would be lost for not conducting a pinhole visual acuity test on each eye and each and every patient. In my experience of reading many clinical records of other practitioners in my role as an expert witness, I can see that it is rarely performed by qualified clinicians.

The pinhole occluder is an opaque disc with one small hole through it. It can be found in every trial set of lenses (unless it has been lost) and every phoropter.

When a patient is asked to view a letter chart through the pinhole the effects of the refractive errors are temporarily removed. Those defects in the optical system that cause refractive error have no effect while the pinhole occluder is used.

The difference between the visual acuity with the current glasses (or no glasses) and the visual acuity through the pinhole occluder can estimate the maximum improvement in a patient’s visual acuity that can be attained by optimum spectacle lenses.

The pinhole occluder can also be used to distinguish visual acuity reduction caused by refractive error, which improves when the occluder is used, from other eye problems, when the pinhole visual acuity does not improve.

For conditions such as age-related macular degeneration a central media opacity, that is conditions affecting the central part of the visual system at the ocular level, the pinhole visual acuity may be worse than the visual acuity without the pinhole. This is because those rays of light which could have helped the patient work out what the image was are cut-off by the occluder, leaving only the central rays which fall on dysfunctional retina or are blocked by a central media opacity.

The pinhole occluder can also be used in testing visual acuity in patients who have been dilated. In this case, the pinhole occluder compensates for the inability to contract the iris assisting the eye in obtaining a retinal projection similar to that of an eye that is not dilated.

I think the short answer is to use the pinhole occluder if you are not sure about the cause of a substandard level of visual acuity. If the substandard level increases through the pinhole then the cause is a less than optimum refractive correction. If the substandard level remains the same or decreases through the pinhole, then the cause is disease-related. Bear in mind that the disease may be subtle or even impossible to see with practice-based techniques.

    I have one question. If ocular media is clear how much refractive error patient having visual acuity improve with pinhole.

      Thank you for your question. All patients with healthy eyes should be able to manage 6/6 and some 6/5. Pinhole visual acuity is not affected by the level of refractive error. A person who is -15.00 DS should manage 6/6-6/5 the same as a person with -1.50 DS. I hope this answers your question.
